A PCMH establishes the primary care physician’s office as the patient’s medical home and is founded on the patient/provider relationship, where the PCP and clinical team members coordinate all of a patient’s medical care to improve patient health. In Michigan, the largest network of PCMH practice designations are made by Blue Cross Blue Shield of Michigan.
