Office Policies

Prescription Refills:

Refills can be optioned by calling our office and selecting #376.  Leave a detailed message with your full name date of birth, medication and pharmacy information. Please make sure to leave your contact information.

Please allow 48 hour for the refill request to be completed.

HMO Referrals:

After receiving your referral from our doctor please contact your specialist and book your appointment.  Leave a message on our Referral Option #312 with your name, date of birth, telephone number, the specialist name, and the date of your appointment.

Please allow up to 1 week for a referral to be processed and authorized by your HMO.

Test Results:

We ask that you allow at least 48 hours from the date of the test for us to attain and review the results.

New Patient Checklist

  • Please download the New Patient Paperwork Forms (located in the sidebar on the right) and bring the completed forms to your appointment.
  • Please arrive 15 minutes before your appointment.
  • Remember to bring your photo ID and insurance card.
  • New patient appointments are scheduled to allow the patient time to establish care with the provider.

Routine Appointments and Extended Hours

Routine Appointments & Extended Hours: Routine appointment scheduling, general information and prescription refill requests will be handled during routine office hours. Same day, early morning and weekend appointments are available. Simply call us to arrange an appointment time that is convenient for you.
