Immunization Records

It is recommended to have an annual flu shot. The seasonal flu vaccine is the best way to protect yourself and others from the flu virus.

Ensure your tetanus vaccine is up to date with a booster dose every 10 years

If you are age 50 or older you may be eligible for the Shingles vaccine. Shingles is a virus that may cause a painful rash and the pain may last for months or years.

Please discuss with your healthcare provider if you are eligible for a pneumococcal vaccine.

Please remember that if you are a new patient to our office to bring your immunization records with you to your first appointment.
